Filling out paperwork when you arrive at your doctor's office is always a lengthy and time consuming process.

My goal for this site is to easily provide you with the electronic files you need to take to your physician so they can put your personal data into their electronic medical records. This is a win for both the patient and the physician.

This site will provide you with several different types of files all included in one secure and compressed file. Included in the file will be your patient paperwork in several different formats for you and your physician.

Under Construction

The quick demographics and all new patient paperwork are under construction. This is a new site but the contact us form works if you would like a quote on a custom web user interface, patient portal, or server and software hosting. We look forward to serving your HL7 needs.
